The original document was fragile and in four pieces so I put them together as best I could and scanned as one document. The names and dates have been blurred for privacy.

When filling in the missing pieces I made educated and calculated guesses as to what would be there. I had nothing other than the one document to pull from. I removed the names and dates on the uploaded finished image to better display the restoration. They are still on the client's image.
